Once prospect fills the form and submit, client will get the form in email with the answers .

== Description ==

Client will get the form in email with the answers and there will be a link in the admin panel to view the survey.

Survey Form display by [ONLINE_SURVEY]

The form will be a page on the site.

list of survey look like that:

Name, Email, View Survey

Admin panel:
can create/edit Categories (category have name and desc)

can create/edit questions. question can have 4 types:
qty
scale
yes/no
text box (one line)

form with details in the bottom (all mandatory except the message field)

you can order the categories.

== Installation ==

1. Upload `online-survey` to the `/wp-content/plugins/` directory
2. Activate the plugin through the 'Plugins' menu in WordPress
3. Place [ONLINE_SURVEY] or  `<?php echo online_survey_template(); ?>` in your templates
