Extract the zip file and just drop the contents in the wp-content/plugins/ directory of your WordPress installation and then activate the Plugin from Plugins page then
Go to  Settings --> Custom content after or before of post

1. Go to  Settings --> MD Custom content after or before of post
2. Input your HTML that you want to display it after or before of post or both
3. Save 
4. Go to a single post to see it
