Create custom appointment form using visual Form Builder & display anywhere using shortcode. You can get a full list of submitted form on admin panel. You can approve/cancel an appointment from plugin settings. 

Plugin provides an visual form builder feature on plugins settings page that provide drag and drop form building options. 

No need of any coding to build any form that you require.

== Installation ==

Extract the zip file and just drop the contents in the wp-content/plugins/ directory of your WordPress installation and then activate the Plugin from Plugins page.

Ans: Yes. You have to put [afm_contact] to display appoint form on any page.

Ans: Yes. You can add css using custom css editor from admin panel.
