In reviewer steps, derive the review scope from the current change request and problems already confirmed for remediation.

- In the initial review, inspect every presented change target and inspect paths affected by the same cause and observable condition in the same round. Do not treat names, types, or file proximity alone as proof that problems are the same.
- In a post-remediation review, first confirm from the current adjudication and repair records the conditions selected for repair and their repair boundary. Inspect unresolved parts, completed repairs, and paths directly affected by those repairs, and connect each finding to one of those conditions or to the current diff. You may report regressions caused by the repair, consumers that must migrate for a changed contract to work, and unvisited consumers that violate the same condition as an already accepted problem. For each new finding, record the adjudication or repair record used as its basis, why it belongs to this scope, and the path you verified. Do not restart general discovery in unchanged areas or expand into adjacent improvements. Apply this scope limit to warnings and recommendations as well as findings that request remediation.
- For a problem first identified during a post-remediation review, use the before-and-after code or records to distinguish an unvisited path that already violated the same condition before repair from a regression introduced by the current repair. If the evidence cannot distinguish them, state what remains unverified instead of guessing.
- Consolidate problems only when their cause, violated observable condition, and reason to change are the same. Keep problems separate when they protect different conditions even if they share a responsible source.
- If post-remediation reviews repeatedly find another location that violates the same observable condition for the same cause after the problem was reported as fixed, do not treat that as evidence of convergence. Treat the earlier impact-path inspection as incomplete and recheck the actual paths related to that problem.
- Do not include a technically valid problem in remediation when its necessity cannot be derived from the current change request or a previously accepted remediation target.

The review mode is `{var:review_mode}`. Treat `initial` as an initial review and `follow_up` as a post-remediation review. Only for `unspecified`, treat a directly executed reviewer step with execution count `{step_iteration}` as initial at 1 and post-remediation at 2 or later. If the mode cannot be determined, use the post-remediation boundary and do not restart general discovery.
