**Existing-finding lookup:**

Before recording a new finding, inspect the latest review reports and review-resolution.md under the Report Directory. When an existing problem has the same cause, violated condition, and acceptance criteria, reuse its `finding_id` and add the location and evidence checked now. Even when a different location is newly observed, keep it in one finding when its cause and acceptance criteria are the same instead of inventing a separate problem name.
