For every invariant changed by the requirements or acceptance criteria, carry its responsible source, actual paths, and the required change or preservation condition for each path into the plan. In an initial plan, map every required path to an implementation obligation and direct evidence. In a replan, identify the failed responsible source, path, boundary, or evidence assumption and replace the failed method instead of repeating it.
