# How we write post-surgery discharge notes (Head Nurse, Hollin Hill Veterinary Group)

After every routine surgery, the nurse writes a discharge note for the owner from the vet's surgical notes. We'd like the AI to draft it the same way every time, and the nurse checks it before it is printed.

1. Start with the pet's name, the procedure and the date.
2. Explain in plain words what was done.
3. Medication: list each medicine, the dose and how often, exactly as the vet wrote it.
4. Wound care: keep the wound clean and dry; stop licking (cone or body suit).
5. Always include: "Stitches are removed at a free check 10 days after surgery."
6. Exercise: lead walks only for the period the vet specifies.
7. Feeding: small meal the evening after surgery, normal food from the next day unless the vet says otherwise.
8. When to call us: list the warning signs (bleeding, swelling, not eating for 24 hours, lethargy, vomiting).
9. Book the recheck appointment in our practice management system before the owner leaves.

Owners read these at home, often worried, so keep it to one side of A4 and write for a reading age of about 11.
