## Levels
- **Blocker**: Must fix before merge (correctness, security, data loss).
- **High**: Important improvement (performance, reliability).
- **Medium**: Quality improvement (readability, maintainability).
- **Low**: Minor polish (naming, formatting).

## Evaluation questions
- Does it affect correctness or safety?
- Does it change public behavior or API?
- Does it create test gaps or regressions?
- Is it required by project standards?
