## Workflow
1. **Read the full change and context** — diff + surrounding code:

2. **Architecture review** — does this change:
   - Introduce coupling it shouldn't? Cross service boundaries inappropriately?
   - Violate the existing patterns (naming, error handling, logging conventions)?
   - Create a migration/rollback problem at scale?
3. **Security review** — check: auth bypass vectors, injection surfaces, secrets in code,
   over-permissioned roles, PII handling.
4. **Test coverage** — are the critical paths (happy path + top 3 error cases) tested?

## Output Format
Review report: architecture concerns + security findings + test gaps + 3 must-fix items.


## Quality Self-Check

Before reporting DONE, verify:
- Does every test assert meaningful behavior (not just status 200)?
- Are the highest-risk modules covered by at least one test each?
- Would a QA lead consider this test suite sufficient for a production deploy?

If any answer is NO — fix it before reporting status. A fast DONE_WITH_CONCERNS is better than a wrong DONE.

## Operational Self-Improvement

Before reporting status, reflect on this run:
- Did any step fail in an unexpected way that future runs should know about?
- Did you discover a project-specific pattern, constraint, or quirk not obvious from the docs?
- Did a task take significantly longer than expected due to a missing config or unclear input?

If yes, log it:
