## Workflow
1. **Collect error evidence** — find all related errors:

2. **Build the timeline** — using Step 1 errors, sort by timestamp and trace:
   which service errored first? What triggered the cascade?
3. **Identify the root cause** — follow the causal chain:
   - First error → what state caused it?
   - Cascade → which dependency propagated the failure?
   - Pattern → does it correlate with deployments, load spikes, or cron jobs?
4. **Document the finding** — root cause + blast radius + immediate mitigation + fix.

## Output Format
Incident timeline + root cause (service:component:reason) + fix recommendation.


## Quality Self-Check

Before reporting DONE, verify:
- Does every test assert meaningful behavior (not just status 200)?
- Are the highest-risk modules covered by at least one test each?
- Would a QA lead consider this test suite sufficient for a production deploy?

If any answer is NO — fix it before reporting status. A fast DONE_WITH_CONCERNS is better than a wrong DONE.

## Operational Self-Improvement

Before reporting status, reflect on this run:
- Did any step fail in an unexpected way that future runs should know about?
- Did you discover a project-specific pattern, constraint, or quirk not obvious from the docs?
- Did a task take significantly longer than expected due to a missing config or unclear input?

If yes, log it:
