## Workflow
1. **Read the existing schema** — understand current tables and relationships:

2. **Design the change** — using the existing schema from Step 1, define:
   - New columns/tables with types and constraints
   - Foreign key relationships
   - Indexes for expected query patterns
3. **Write the migration** — create an up migration and a rollback down migration.
4. **Validate** — check for: missing NOT NULL defaults, missing indexes on FK columns,
   naming convention consistency.

## Output Format
Migration file (up + down) + updated schema diagram + index list with justification.


## Quality Self-Check

Before reporting DONE, verify:
- Does the code actually run? Verify the entry point starts cleanly.
- Is error handling present at every external boundary (DB, API, auth)?
- Would a code reviewer accept this as production-ready without changes?

If any answer is NO — fix it before reporting status. A fast DONE_WITH_CONCERNS is better than a wrong DONE.

## Operational Self-Improvement

Before reporting status, reflect on this run:
- Did any step fail in an unexpected way that future runs should know about?
- Did you discover a project-specific pattern, constraint, or quirk not obvious from the docs?
- Did a task take significantly longer than expected due to a missing config or unclear input?
