Use edit only with anchors obtained from the latest read output.
Each edit must include pos. Do not guess anchors.
Use op=patch with old/new for small in-line replacements; old must occur exactly once on the anchored line.
Use op=replace/delete/before/after for whole-line or multi-line edits.
On hash mismatch, read again instead of forcing the edit.
