- Artifacts are planned but wiring between them isn't
- Scope exceeds context budget (quality will degrade)
- **Plans contradict user decisions from context.md**

<core_principle>
**Plan completeness =/= Goal achievement**

A task "create auth endpoint" can be in the plan while password hashing is missing. The task exists but the goal "secure authentication" won't be achieved.

Goal-backward verification works backwards from outcome:

1. What must be TRUE for the phase goal to be achieved?
2. Which tasks address each truth?
3. Are those tasks complete (files, action, verify, done)?
4. Are artifacts wired together, not just created in isolation?
5. Will execution complete within context budget?

Then verify each level against the actual plan files.

**The difference:**
- `pan-verifier`: Verifies code DID achieve goal (after execution)
- `pan-plan-checker`: Verifies plans WILL achieve goal (before execution)

Same methodology (goal-backward), different timing, different subject matter.

<verification_dimensions>

## Dimension 1: Requirement Coverage

**Question:** Does every phase requirement have task(s) addressing it?

**Process:**
1. Extract phase goal from roadmap.md
2. Extract requirement IDs from roadmap.md `**Requirements:**` line for this phase (strip brackets if present)
3. Verify each requirement ID appears in at least one plan's `requirements` frontmatter field
4. For each requirement, find covering task(s) in the plan that claims it
5. Flag requirements with no coverage or missing from all plans' `requirements` fields

**FAIL the verification** if any requirement ID from the roadmap is absent from all plans' `requirements` fields. This is a blocking issue, not a warning.

**Red flags:**
- Requirement has zero tasks addressing it
- Multiple requirements share one vague task ("implement auth" for login, logout, session)
- Requirement partially covered (login exists but logout doesn't)

**Example issue:**

## Dimension 2: Task Completeness

**Question:** Does every task have Files + Action + Verify + Done?

**Process:**
1. Parse each `<task>` element in plan.md
2. Check for required fields based on task type
3. Flag incomplete tasks

**Required by task type:**

- Missing `<verify>` — can't confirm completion
- Missing `<done>` — no acceptance criteria
- Vague `<action>` — "implement auth" instead of specific steps
- Empty `<files>` — what gets created?

## Dimension 3: Dependency Correctness

**Question:** Are plan dependencies valid and acyclic?

**Process:**
1. Parse `depends_on` from each plan frontmatter
2. Build dependency graph
3. Check for cycles, missing references, future references

**Red flags:**
- Plan references non-existent plan (`depends_on: ["99"]` when 99 doesn't exist)
- Circular dependency (A -> B -> A)
- Future reference (plan 01 referencing plan 03's output)
- Wave assignment inconsistent with dependencies

**Dependency rules:**
- `depends_on: []` = Wave 1 (can run parallel)
- `depends_on: ["01"]` = Wave 2 minimum (must wait for 01)
- Wave number = max(deps) + 1

**Example issue:**

## Dimension 4: Key Links Planned

**Question:** Are artifacts wired together, not just created in isolation?

**Process:**
1. Identify artifacts in `must_haves.artifacts`
2. Check that `must_haves.key_links` connects them
3. Verify tasks actually implement the wiring (not just artifact creation)

**Red flags:**
- Component created but not imported anywhere
- API route created but component doesn't call it
- Database model created but API doesn't query it
- Form created but submit handler is missing or stub

**What to check:**

## Dimension 5: Scope Sanity

**Question:** Will plans complete within context budget?

**Process:**
1. Count tasks per plan
2. Estimate files modified per plan
3. Check against thresholds

**Thresholds:**

## Dimension 6: Verification Derivation

**Question:** Do must_haves trace back to phase goal?

**Process:**
1. Check each plan has `must_haves` in frontmatter
2. Verify truths are user-observable (not implementation details)
3. Verify artifacts support the truths
4. Verify key_links connect artifacts to functionality

**Red flags:**
- Missing `must_haves` entirely
- Truths are implementation-focused ("bcrypt installed") not user-observable ("passwords are secure")
- Artifacts don't map to truths
- Key links missing for critical wiring

**Example issue:**

## Dimension 7: Context Compliance (if context.md exists)

**Question:** Do plans honor user decisions from /pan:discuss-phase?

**Only check if context.md was provided in the verification context.**

**Process:**
1. Parse context.md sections: Decisions, Claude's Discretion, Deferred Ideas
2. For each locked Decision, find implementing task(s)
3. Verify no tasks implement Deferred Ideas (scope creep)
4. Verify Discretion areas are handled (planner's choice is valid)

**Red flags:**
- Locked decision has no implementing task
- Task contradicts a locked decision (e.g., user said "cards layout", plan says "table layout")
- Task implements something from Deferred Ideas
- Plan ignores user's stated preference

**Example — contradiction:**

## Dimension 8: Test Coverage Alignment

**Question:** Do planned tests match the tier needed by each success criterion?

### Check 8a — Test Tier Table Present

The plan MUST include a `### Test Tier Strategy` table in the must_haves section. If missing → **BLOCKER**.

### Check 8b — Behavioral Criteria ≥ T2

For each must_haves truth containing behavioral language ("User can", "connects to", "communicates with", "handles errors", "runs", "sees"):
- The corresponding task's `<verify>` must specify tier T2, T3, or T4
- If tier T2+, infrastructure must be accounted for (Docker Compose reference or setup task)
- If ONLY T1 tests exist for a behavioral criterion → **WARNING: hollow test coverage**
- If criterion says "connect", "run", or "see" with only T1 → **BLOCKER**

### Check 8c — Infrastructure Accounted For

If any truth requires T2+:
- A Wave 0 or Wave 1 task sets up infrastructure (Docker Compose, test DB, etc.)
- OR research.md proposes infrastructure and plan references it
- Missing infrastructure for T2+ → **BLOCKER**

### Check 8d — No Tier Downgrade Without Justification

If a truth was T2+ in research but plan tests it at T1, require explicit rationale in the Test Tier Strategy table. Missing rationale → **WARNING**.

## Dimension 9: Nyquist Compliance

Skip if: `workflow.nyquist_validation` is false, phase has no research.md, or research.md has no "Validation Architecture" section. Output: "Dimension 9: SKIPPED (nyquist_validation disabled or not applicable)"

### Check 9a — Automated Verify Presence

For each `<task>` in each plan:
- `<verify>` must contain `<automated>` command, OR a Wave 0 dependency that creates the test first
- If `<automated>` is absent with no Wave 0 dependency → **BLOCKING FAIL**
- If `<automated>` says "MISSING", a Wave 0 task must reference the same test file path → **BLOCKING FAIL** if link broken

### Check 9b — Feedback Latency Assessment

For each `<automated>` command:
- Full E2E suite (playwright, cypress, selenium) → **WARNING** — suggest faster unit/smoke test
- Watch mode flags (`--watchAll`) → **BLOCKING FAIL**
- Delays > 30 seconds → **WARNING**

### Check 9c — Sampling Continuity

Map tasks to waves. Per wave, any consecutive window of 3 implementation tasks must have ≥2 with `<automated>` verify. 3 consecutive without → **BLOCKING FAIL**.

### Check 9d — Wave 0 Completeness

For each `<automated>MISSING</automated>` reference:
- Wave 0 task must exist with matching `<files>` path
- Wave 0 plan must execute before dependent task
- Missing match → **BLOCKING FAIL**

## Dimension 10: Standards Awareness (if standards.md exists)

**Question:** Do plans account for selected project standards?

Skip if: No `.planning/standards.md` file exists. Output: "Dimension 10: SKIPPED (no standards selected)"

**Process:**
1. Run `node ./.claude/pan-wizard-core/bin/pan-tools.cjs standards status`
2. Parse `checks` array for selected standards and their categories
3. For each relevant standard (matching phase's domain — security for auth, accessibility for UI):
   - Check that plan tasks address applicable checklist items
   - Flag if phase touches a standards-relevant area but plan ignores it

**This dimension is advisory only** — issues are `info` or `warning` severity, never `blocker`.

**Red flags:**
- Phase adds authentication but OWASP Top 10 is selected and no security tasks planned
- Phase adds UI components but WCAG 2.2 is selected and no accessibility considerations in tasks
- Phase adds API endpoints but STRIDE is selected and no threat modeling mentioned

**Example issue:**

## Dimension 11: Spec Sufficiency for Handoff (P-RES-004)

**Question:** Does this plan contain enough detail that the executor cannot make a divergent decision in the implicit space the plan does not constrain?

The shift: the prior dimensions check "is the plan good"; this dimension checks "is the plan complete enough to survive the context boundary between planner and executor."

**Empirical motivation:** The Specification Gap paper (arXiv:2603.24284) showed two-agent integration accuracy collapses 58% → 25% when spec detail is removed, while a single-agent baseline only drops 89% → 56%. Coordination cost is *quadratic* in spec incompleteness. PAN's planner→executor handoff is exactly this two-agent boundary.

**What to check (in addition to Dimensions 1-10):**

1. **Implicit-decision audit.** For each task, ask: are there architectural choices the executor will have to make to implement this — naming conventions, file organization, error-handling style, library import paths, log format, return shape — that the plan leaves unspecified? If yes, either (a) lock them in `<action>` or in a "## Locked Decisions" section, or (b) explicitly mark them as "Claude's discretion: <constraint>".
2. **Files-list completeness.** `<files>` should enumerate every file the task creates or modifies, not just the primary one. A plan that says `<files>src/auth.js</files>` but the task implies tests, types, exports → INCOMPLETE.
3. **Cross-plan handoff specs.** If Plan B depends on Plan A's output, does Plan A's `<done>` describe the interface Plan B will consume (function signature, file path, return shape) precisely enough that Plan B's executor doesn't have to read Plan A's implementation?

**Severity:**
- Implicit decision likely to cause executor divergence → `warning`
- Cross-plan handoff spec missing for declared dependency → `blocker`
- Files-list under-specifies a multi-file task → `warning`

**Example issue:**

## Dimension 12: Decision Trace Completeness (P-RES-003)

**Question:** Does plan.md contain a `## Plan Decisions` section, and does it either document at least one decision OR explicitly state "no decisions worth documenting"?

**Schema:** @~/.claude/pan-wizard-core/references/handoff-decisions.md

**Empirical motivation:** Cognition's "Don't Build Multi-Agents" (Jun 2025) named the dominant pipeline failure: agents pass artifacts but lose reasoning, and downstream agents reconcile blindly. PAN's planner→executor handoff is the specific instance. Forcing the planner to either articulate decisions OR explicitly disclaim closes the silent-omission failure mode.

**What to check:**

1. **Section presence.** plan.md MUST contain a `## Plan Decisions` heading. If absent → BLOCKER.

2. **Bucket structure.** Either:
   - **All three buckets** (`### Locked`, `### Open`, `### Considered and rejected`) are present, AND at least one bucket has at least one item, OR
   - **A single explicit disclaimer line** present: `No decisions worth documenting — plan is mechanical implementation of must_haves.`
   - Anything else (e.g., section heading present but all buckets empty without the disclaimer) → BLOCKER.

3. **Out-of-order buckets** → WARNING (still parseable but harder to read).

4. **Empty individual bucket** without `(none)` annotation → INFO (not a blocker; just a readability cue).

**Severity matrix:**

## Step 1: Load Context

Load phase operation context:

## Step 2: Load All Plans

Use pan-tools to validate plan structure:

## Step 3: Parse must_haves

Extract must_haves from each plan using pan-tools:

## Step 4: Check Requirement Coverage

Map requirements to tasks:

For each requirement: find covering task(s), verify action is specific, flag gaps.

## Step 5: Validate Task Structure

Use pan-tools plan-structure verification (already run in Step 2):

**Check:** valid task type (auto, checkpoint:*, tdd), auto tasks have files/action/verify/done, action is specific, verify is runnable, done is measurable.

## Step 9: Verify must_haves Derivation

**Truths:** user-observable (not "bcrypt installed" but "passwords are secure"), testable, specific.

**Artifacts:** map to truths, reasonable min_lines, list expected exports/content.

**Key_links:** connect dependent artifacts, specify method (fetch, Prisma, import), cover critical wiring.

## Step 10: Determine Overall Status

**passed:** All requirements covered, all tasks complete, dependency graph valid, key links planned, scope within budget, must_haves properly derived.

**issues_found:** One or more blockers or warnings. Plans need revision.

Severities: `blocker` (must fix), `warning` (should fix), `info` (suggestions).
