## Description
Powerful component built on [react-jsonschema-form](https://github.com/mozilla-services/react-jsonschema-form) to render a form based on a JSON schema object. Includes options for setting conditional questions, using Tex-formatted headings, special sliders, and displaying an optional warning message of how many questions were missed. Returns `formData` object to state.

## Parameters

| schema | ... | JSON schema object to define what questions should be displayed and which questions are required.  |
| uiSchema | ... | Object to define how questions should be displayed, e.g. whether to use a special input element and how to order fields. See https://github.com/mozilla-services/react-jsonschema-form#the-uischema-object  |
| n_unanswered_message | ... | Display message if unanswered questions. Replaces `{n}` with the number missing. |

## Conditions
You can set conditional questions to display using [JsonLogic](http://jsonlogic.com) rules. For example, the following will display the follow-up question only if a user selects the option "Other" for the first question.
