## 1.65.0 (2026-06-15)

### Added

- **Ambient session memory capture** (Proposal 12, P1). Claudia now forms memories from the conversation itself, without anyone having to invoke a memory tool. A `SessionStart` disk-scan (crash-proof) and a `SessionEnd` fast-path enqueue finished sessions to `~/.claudia/sessions_pending.jsonl`; a new daemon `process_sessions` job parses the transcript, extracts facts, commitments, and decisions via the local Ollama ingest service, and writes them through an AUDN (add/update/no-op) pass that dedupes against existing memories. The daemon is the sole writer and the hooks never touch SQLite, so it cannot reintroduce the WAL-contention class. The AUDN update validates the target id against the candidate set (a hallucinated id can never overwrite an unrelated memory) and preserves superseded content in `metadata.corrected_from`. New files: `memory-daemon/claudia_memory/services/audn.py`, `template-v2/.claude/hooks/session-enqueue.py`. Defaults on; set `session_capture_enabled` to false to disable.
- **Session-start "update available" notice.** When a newer `get-claudia` has shipped, Claudia surfaces it at session start (for example, "Update available: Claudia v1.66.0, run claudia update"). The check is cached for 24 hours, timeout-bounded, and fail-open, so it never slows or blocks startup. This covers the gap where the launcher only update-checks when you run the installer, not when you start a session.

## 1.64.0 (2026-06-13)

### Added

- **Status-only wrap for daemon scheduled jobs** (Proposal 11, E5). The seven background jobs (importance decay, pattern detection, consolidation, daily and weekly backups, vault sync, observation ingest) now each write `~/.claudia/loops/<job>_status.md` and check deterministic invariants (the backup jobs assert the backup file exists and is non-empty). It is status-only: a failed invariant is flagged but never halts the job, and a job that raises is recorded and then re-raised, so daemon behavior is unchanged. `memory.system_health` and the `/status` endpoint now surface each job's last verdict plus a flagged count. New module `claudia_memory/loops/job_wrapper.py` (6 tests); health surfacing has 2 tests; full daemon suite stays green (803 passed). This completes Proposal 11.

## 1.63.0 (2026-06-13)

### Added

- **`/build-team` skill** (Proposal 11, E6). A user-invoked counterpart to `hire-agent`: it reads the user's profile, judgment rules, and real task history, then proposes a minimal, tailored agent team in one pass. The proposal runs through the independent `loop-checker` (scored against goal alignment, right-sizing, and a "progressive, not overwhelming" hard constraint, bounded to 2 revisions), is written to a `team_status.md` control file, and is gated on explicit user approval. On approval it scaffolds agent definitions and writes `.bak` siblings for any modified file so the change is reversible. Reuses the existing roster first and never auto-spawns agents or takes external actions. New files: `template-v2/.claude/skills/build-team/SKILL.md` (+ `skill-index.json` entry).
- **Proactive team review** (Proposal 11, E7). `hire-agent` now distinguishes "add one agent" from "the whole team has drifted." When it detects an archetype shift, a new recurring task class the roster does not cover, or two-plus unused agents, it gently suggests reviewing the team as a unit and routes to `/build-team` for the validated, approval-gated, reversible change. One suggestion at a time; never auto-applied; a review can shrink a team as readily as grow it.

## 1.62.0 (2026-06-13)

### Loop engineering foundation (Proposal 11, Phases 1-2)

The first two phases of the Autonomy & Personalization Layer: the Maker-Checker pattern on the existing `auto-research` loop, a bounded self-repair sub-loop, the shared loop infrastructure later loops reuse, and a `meditate` feed that reviews how the loops themselves performed. See `docs/proposals/11-autonomy-personalization-layer.md`.

### Added

- **Independent Checker for `auto-research`** (Proposal 11, E2). The iteration loop no longer grades its own work. Each iteration now dispatches a new `loop-checker` agent (Haiku, adversarial brief) that scores the artifact against the rubric independently; the Checker's score, not Claudia's, drives the keep/revert decision. When the Checker's score and Claudia's self-score diverge past threshold, the iteration is flagged `contested` and surfaced in the end-of-run summary. New files: `template-v2/.claude/agents/loop-checker.md` and `template-v2/.claude/skills/_loop/` (`maker.md`, `checker.md`, `README.md`).
- **Loop status-file schema** (Proposal 11, E1). `docs/loop-status-schema.md` defines a standard Markdown-body + YAML-frontmatter control plane (`last_input`, `maker_proposal`, `checker_verdict`, `verified`, `next_action`, and friends) plus the exit-condition standard every loop must declare up front. `auto-research` now writes `research_status.md` in this format each iteration.
- **Atomic status-file helper** (Proposal 11, E1/B3). New daemon module `claudia_memory/loops/status.py` (`write_status` / `read_status`) writes a status file through a same-directory temp file plus `os.replace`, so an interrupted write never leaves a partial file at the canonical path. Six tests in `memory-daemon/tests/test_loop_status.py` cover round-trip, native-type preservation, parent-dir creation, temp cleanup, and crash-safety.
- **Self-repair sub-loop** (Proposal 11, E3). When a loop stalls (the Checker fails 3 times running or scores regress), `auto-research` now enters a bounded self-repair pass (`template-v2/.claude/skills/_loop/repair.md`): it diagnoses whether the rubric or a brief is the real problem, proposes one fix, and proves it on the exact failing input before adopting it. Capped at 2 repair attempts; confirmed repairs leave a regression fixture under `~/.claudia/loops/regressions/`; edits to shipped briefs always go to a human approval gate.
- **`meditate` reviews loop-harness performance** (Proposal 11, E4). End-of-session reflection now reads recent loop status files, summarizes how Claudia's own Maker-Checker loops did (contested iterations, stuck loops, recurring Checker findings), and can propose a harness improvement. Any such proposal passes through the Checker before it is written, and edits to shipped briefs are presented as a diff for approval.

## 1.61.1 (2026-05-23)

### `claudia` command works from anywhere, and repairs itself

A follow-up to 1.61.0's shell command. The installer was storing a relative path, so `claudia` only worked from the install's parent directory. This release fixes the stored path and makes the command self-healing, so the error cannot recur.

### Fixed

- **`claudia` command failing with "Claudia home directory not found: claudia".** The installer wrote a *relative* path (`claudia`) into `~/.claudia/claudia-home` instead of the absolute install path, because the shell-helper step was handed `targetDir` (relative) rather than the resolved `targetPath`. The `claudia` command then only worked when run from the install's parent directory and failed everywhere else. The installer now always stores the absolute path (via `path.resolve`, which also fixes a separate bug where passing an absolute install path as an argument produced a doubled path like `/home/you/home/you/claudia`).
- **`claudia` command now self-heals a bad `claudia-home`.** The shell function resolves a relative stored value against `$HOME` (never the current directory), falls back to the default `~/claudia` install when the stored path is missing or stale, and rewrites `claudia-home` with the corrected absolute path so the error cannot recur. Existing installs that already have a bad value are repaired the first time `claudia` runs after upgrading.
- **`--skip-memory` installs now get the `claudia` command too.** Previously the shell-helper step was skipped entirely on the `--skip-memory` path, leaving those users with no `claudia`/`update-claudia` command. The shell helper is independent of the memory system and now installs on every path.

## 1.61.0 (2026-05-22)

### Launch from anywhere, upgrade from anywhere

This release adds a `claudia` shell command (and `update-claudia`) so you no longer have to remember where you installed Claudia or `cd` there before launching or upgrading. New installs and upgrades wire the helper into `~/.zshrc` and `~/.bashrc` automatically. After upgrading once via `npx get-claudia .`, future upgrades are just `update-claudia` from any directory.

### Added

- **`claudia` shell command for launching from anywhere.** New installs (and upgrades) get a `claudia` shell function written to `~/.claudia/shell-init.sh` and sourced from `~/.zshrc` and `~/.bashrc`. From any directory, `claudia` cd's to your install folder and launches `claude`. All `claude` flags pass through (`claudia --resume`, `claudia -c "morning brief"`, etc). The folder path is stored in `~/.claudia/claudia-home`; edit it if you move your install. The installer is idempotent: re-running won't double-add to your rc files. The shell function is a smart router that delegates known npm-CLI subcommands (`setup`, `system-health`, `google`, `doctor`) back to the `claudia` binary when it's on PATH, so existing workflows keep working.
- **`claudia yolo` subcommand.** Sugar for `claude --dangerously-skip-permissions` (cd's first, then passes through any additional flags). Same routing logic — `claudia yolo --resume` works.
- **`update-claudia` command (also `claudia update`).** Runs `npx get-claudia <your-install-path>` from any directory by reading the stored path from `~/.claudia/claudia-home`. No more "wait, where did I install Claudia?" before upgrading. Both forms are aliases for the same logic.
- **`~/.claudia/claudia-home`.** Single-line file with the absolute path of the user's Claudia install. Written by the installer; read by the shell function. Move your install? Edit this file.

### Fixed

- **ASCII greeting logo: row 1 (hair top) realigned over face** (in `template-v2/CLAUDE.md`). The top hair row was indented 6 leading spaces while the face row below it starts at column 0, so the hair appeared shifted ~4 characters to the right. Now sits correctly above the face. Existing installs see the fix on next upgrade.
- **Installer no longer treats flag-looking arguments as install targets.** Previously, `npx get-claudia --help` (or any unrecognized `--flag`) was interpreted as the install path and would create a `./--help/` directory with templates copied into it. Now: `--help`/`-h` prints a proper usage message and exits, `--version`/`-V` prints the version, and any other leading-dash argument prints `Error: unrecognized argument: <flag>` and exits 1 without touching the filesystem.
## 1.60.1 (2026-05-22)

### Two bug fixes from the fixing-phase pass

Both shipped together as a patch release. No new features; no surface-area change. Upgrade with `npx get-claudia .` from any existing install.

### Fixed

- **Memory daemon: WAL checkpoint no longer blocks concurrent readers** (#66, thanks @tilthnco). `database.py` now uses `PRAGMA wal_checkpoint(PASSIVE)` on every connection instead of `TRUNCATE`. `TRUNCATE` takes an exclusive lock, which deadlocks against concurrent WAL readers like Litestream and causes the MCP server to time out after 30s on startup. `PASSIVE` yields cleanly if a reader holds the WAL lock. Compaction behavior is unchanged; only the locking mode is relaxed. Crash safety is unaffected since WAL mode (not checkpointing) is the durability mechanism.
- **Memory daemon: organisations from session summaries no longer misclassified as `person`** (Proposal #51, sub-tranche B2). Two stale defaults were bypassing the smart type-inference path: `RememberService.end_session()` hard-defaulted missing `type` fields to the literal string `"person"` (short-circuiting inference inside `remember_entity()`), and `remember_entity()` itself was still calling the legacy local `_infer_entity_type()` (returns `"person"` fallback, no `"AI"`-suffix rule) instead of the smart inferencer at `entities.infer_entity_type` (returns `"concept"` fallback, handles `"Markup AI"`, `".ai"`, etc.). Both call sites now route through the smart inferencer, matching `_find_or_create_entity()` which already did. The literal repro from 2026-05-13 (Matt Blumberg, Markup AI) now classifies Markup AI as `organization`. See `docs/proposals/08-smarter-memory-writes.md` for the original proposal; `tests/test_entity_resolution.py::TestEndSessionInfersEntityType` pins the fix (3 new tests, 25/25 in the file pass).

### Changed

- 27 skill descriptions in `template-v2/.claude/skills/` now end with a "See also: …" pointer to adjacent skills (from #61). Affected clusters: outbound message composition, memory introspection, memory visualization, reflective cadences, meeting lifecycle, risks and blind spots, people and relationships, patterns and judgment, inbound document processing. Skill names, trigger phrases, and behaviors are unchanged; descriptions are extended additively to nudge skill routing toward the canonical pick when a request straddles two skills.

## 1.60.0 (2026-05-15)

### Three new skills inspired by Karpathy's recent work, adapted to Claudia's principles

This release adds the wiki layer (new default vault projection, inspired by Karpathy's [llm-wiki](https://gist.github.com/karpathy/442a6bf555914893e9891c11519de94f)), the auto-research skill (workspace-scoped iteration loop, inspired by Karpathy's [autoresearch](https://github.com/karpathy/autoresearch)), and the skill router (discovery and disambiguation meta-skill for the catalog).

### Wiki layer (new default vault projection)

The wiki is the third tier of Claudia's memory: raw memories live in the database, derived signals (entities, reflections, patterns) live in the daemon, and **synthesized topic pages** now live in the user's vault at `~/.claudia/vault/Wiki/`. Each page is written by Claudia from raw memories, cites every load-bearing claim with `[mem:NNN]`, flags contradictions at the top, and grows incrementally with use.

- New `wiki` skill at `template-v2/.claude/skills/wiki/SKILL.md` with two reference docs (page template, citations/contradictions discipline). The skill defines when to write pages, the page structure, the citation format, and the read workflow (consult the wiki page first, fall back to memory query if stale).
- Vault default for new installs is now wiki, not PARA. New installs write synthesized pages at `~/.claudia/vault/Wiki/` instead of mechanical entity dumps.
- `vault-awareness` skill updated with a "Wiki vs PARA" section at the top and a detection rule, with a pointer to the new wiki skill for specifics.

**Compatibility:** Existing PARA vaults are preserved untouched. Users with vaults from v1.42 to v1.59 keep their existing structure. Detection rule: if `~/.claudia/vault/Active/` or `Relationships/` exists without `Wiki/`, treat as PARA. The mechanical dump continues to function for them. A future `claudia-memory --migrate-to-wiki` CLI will copy PARA aside and rebuild wiki pages from raw memories. Not in this release; PARA users stay on PARA until they explicitly migrate.

### Auto-research skill (workspace-scoped iteration loop)

Workspace-scoped hill-climbing loop for iterating on local artifacts. Adapted to Claudia's safety principles (workspace-only edits, no external actions during the loop, bounded budget, baseline-score gate, per-iteration revertability).

- New `auto-research` skill at `template-v2/.claude/skills/auto-research/SKILL.md` with two reference docs: `references/program-template.md` for the governance file each run uses, and `references/safety-rules.md` documenting the 8 mandatory safety rules.
- Workspace at `~/.claudia/auto-research/<task-id>/`. Each run gets its own directory with a fresh git repo, an immutable copy of the original artifact, a working copy that gets edited, a `results.tsv` history, and per-iteration snapshots.
- Loop: read state, propose one specific change, implement, score against rubric, ratchet if better OR revert if worse, report to user as one line per iteration.
- Default budget: 20 iterations. Plateau detection: stops early if 5 consecutive iterations show no improvement.
- The user's original file is **never** modified during the loop. Hand-off at the end requires explicit user confirmation of destination.
- Refuse-to-start conditions are explicit: external-action artifacts, sensitive content, no clear evaluator, already-good baseline, or cases where bold structural change is the actual need (not iteration).
- Karpathy named the conservatism ceiling himself: RLHF-trained iteration is "cagy and scared." Iterations tend toward safe edits, not bold reframing. The skill surfaces this honestly when the loop plateaus, so the user knows when to stop iterating and start a fresh draft instead.

### Skill router (discovery + disambiguation)

A meta-skill that helps both users and Claudia navigate the ~42-skill catalog. Addresses two long-standing problems: users not knowing what's available, and Claudia firing the wrong skill when a request straddles two.

- New `skill-router` skill at `template-v2/.claude/skills/skill-router/SKILL.md` with `references/overlap-clusters.md` documenting all 10 known overlap clusters with canonical picks and disambiguation patterns.
- **Discovery surface.** `/skills` returns a categorized list (daily flow, reviews, pipeline, knowledge, drafting, setup). `/skills <topic>` filters by keyword.
- **Disambiguation surface.** When a request matches 2+ skills, Claudia names the candidates briefly and proceeds with the canonical one. Pattern: "Sounds like X or Y. I'll do X. Say so if you wanted Y."

### Not yet in this release
- Automatic wiki-refresh queue (mark entities as "dirty" on ingest, batch refresh later).
- Daemon-side MCP tools for wiki page metadata.
- Shell-command evaluators for auto-research (today: rubric-based scoring only).

## 1.58.0 (2026-05-13)

### The Memory Reliability Release

Five PRs that fix the memory layer's biggest recurring failure mode and lock in the integration philosophy. After this release, memory writes that name entities ("Matt Blumberg") actually create those entities with the correct type. The release history's recurring memory-fix releases ("Recall Recovery", "Vector Search Fix", "Semantic Search Actually Works Now") get permanent regression-test sentinels so the same bug classes can't quietly come back. And the codebase loses a dual-maintenance hazard that was already costing time.

#### Fixed
- **`memory_remember` actually links entities and infers their type correctly (#54)** -- A confirmed bug from 2026-05-13: calling `memory_remember(content="Matt Blumberg said X", entities=["Matt Blumberg", "Markup AI"])` was creating entities but assigning them `type: person` by default, even when the name clearly indicated an organization. "Markup AI" was being saved as a person. The real bug was in `_infer_entity_type` -- it didn't recognise `AI` / `.ai` / `Co.` as corporate suffixes and fell back to `person`. Fixed with a pure-function rule-based type inference (corporate suffixes -> organization, project keywords -> project, person patterns -> person, fallback -> concept, never default to person). Plus a new `claudia-memory --backfill-entities` CLI to retroactively link orphaned references in existing user databases.

#### Added
- **`claudia memory backfill-entities` command (#54)** -- Default dry-run: prints a plan and writes nothing. `--apply` makes a timestamped backup to `~/.claudia/backups/memory-{timestamp}.db` first, then applies the backfill. Idempotent: re-running on an already-backfilled DB is a no-op. Aborts cleanly if backup creation fails.
- **5 regression tests for recurring bug classes (#56)** -- New `memory-daemon/tests/test_recurring_regressions.py` adds permanent forward-looking sentinels for: entity linking on `memory_remember`, recall returning results after seed writes, embedding migration preserving recall, daemon startup tolerating stale SHM files, and `memory_briefing` returning a valid structure on an empty database. Each test docstring names the historical releases where its bug class appeared (v1.35.x, v1.51.5, v1.51.18, v1.55.7, v1.55.8, v1.55.14, v1.21.1, v1.40.1).
- **API parameter aliases for read-side MCP tools (#57)** -- `memory_about` now accepts `entity_name` and `name` alongside `entity`. `memory_relate` accepts `source_entity` / `target_entity` / `relationship_type` alongside `source` / `target` / `relationship`. `memory_recall` accepts `q` and `search` alongside `query`. Purely additive: every existing caller continues to work unchanged. Aliases normalize at the MCP boundary; service-layer signatures are untouched. If both canonical and alias are passed in the same call, canonical wins.

#### Removed
- **Rube (Composio) MCP integration as a bundled default (#41)** -- Rube is no longer a recommended or bundled MCP server in `.mcp.json.example` (root and template-v2), README, or the Claudia documentation. Locks in the direct-integrations-only philosophy (claude.ai-native MCPs + user-built custom MCPs like Gmail/Calendar). Existing users with `rube` already configured continue to work unchanged; the installer simply no longer ships Rube as an example. The "Tool configuration" example in `claudia-principles.md` was updated to vendor-neutral phrasing.
- **Legacy `claudia/` sibling files (#55)** -- Removed 3 stale sibling files (`post-tool-capture.py`, `session-health-check.py`, `settings.local.json`) that lived under `claudia/`. These were never reaching users (the installer ships from `template-v2/` only), but every hook bug fix had to remember to patch both locations. The dual-maintenance hazard was real: PR #38's sibling-fix step had to apply the same env-var fix twice. Removed at the source.

#### Notes
- The bug in #54 was different from the original proposal (#51) described. The proposal said "entities are silently ignored." Actually the entity *records* were getting created -- the bug was that they were all getting `type: person`. Fixing the actual bug rather than the imagined one was a better outcome.
- The `claudia memory backfill-entities` command surface lives on the daemon's argparse (alongside `--backfill-embeddings`, `--migrate-vault-para`), not as a `claudia memory ...` subcommand on the Node CLI. The Node CLI is the installer, not a memory-command dispatcher.
- Aliases are NOT yet advertised in the MCP `list_tools()` `inputSchema`. They are tolerantly accepted at the request boundary. Schema-level advertisement is a future enhancement if it proves needed for client discoverability.

## 1.57.0 (2026-05-13)

### The Curated Memory Release

Five PRs that complete one thesis: **curated, judgment-driven memory capture, enforced at prompt time and persisted across sessions.** Claudia now catches the user's intent when it matters, persists canonical facts as they emerge, and writes a daily session summary so context survives across days.

#### Fixed
- **PostToolUse hook actually runs (#38)** -- The hook was reading `os.environ.get("CLAUDE_TOOL_NAME")`, which Claude Code never sets. Every install since the hook landed had been silently no-op'ing, so `~/.claudia/observations.jsonl` was never written. The hook now reads its payload from stdin per the documented hook contract. Includes a sibling fix to the legacy `claudia/.claude/hooks/post-tool-capture.py` for codebase consistency.

#### Added
- **Memory-commitment rule (#39)** -- A new always-active rule (`template-v2/.claude/rules/memory-commitment.md`) codifies when to save canonical facts immediately via `memory_remember` / `memory_batch` rather than batching to end-of-session reflection. Trigger phrases include "lock this in," "remember this," "this is canonical." Substantive-artifact discipline: at the end of producing a multi-file artifact, do a memory commitment pass and save the canonical facts as one bundled `memory_batch` call.
- **UserPromptSubmit hook with intent detection (#42)** -- A new hook (`template-v2/.claude/hooks/user-prompt-capture.py`) inspects the user's prompt at submit time and injects reminder context for two trigger classes. Class 1: canonical-fact phrases ("lock this in," "remember this," etc.) tell the agent to save immediately rather than wait for `/meditate`. Class 2: destructive command patterns (`rm -rf`, `git push --force`, `DROP TABLE`, etc.) trigger a "verify before acting" reminder per the safety-first principle. Destructive patterns are surfaced to the model as human-readable labels (`rm -rf (recursive delete)`), not raw regex, so the agent can reason about them clearly.
- **Daily session summary system (#40)** -- A new SessionEnd hook (`template-v2/.claude/hooks/session-summary.py`) writes a per-session markdown summary to `~/.claudia/sessions/YYYY-MM-DD/NN-slug.md` covering opening prompt, files touched, external actions, and find-this-again references. SessionStart now surfaces a 3-day digest of recent sessions via the existing health-check hook, so future-Claudia knows what past-Claudia worked on. - **Auto-install Python 3.12** -- On macOS with Homebrew, if only Python 3.14 exists, the installer runs `brew install python@3.12` automatically before creating the venv.
- **LaunchAgent verification** -- After registering the macOS LaunchAgent, the installer now verifies the standalone daemon is actually running via `launchctl list`. Three features adapted from claude-mem research. All additive, no schema changes, no migrations, no new pip dependencies.

- **Privacy tag filtering** -- Content wrapped in `<private>...</private>` tags is stripped before storage. Applied at all three entry points: `remember_fact()`, `remember_message()`, and `buffer_turn()`. Case-insensitive, multiline, non-greedy. If stripping would produce empty content, the original is preserved. Users can now share sensitive context with Claudia while keeping specific details out of the database.
- **Session briefing injection** -- The SessionStart hook now also fetches `http://localhost:3848/briefing` (the daemon's existing HTTP endpoint) with a 3s timeout. When the daemon is running, the briefing data arrives in `additionalContext` before Claude's first response, eliminating the need for a separate `memory.briefing` MCP tool call. Graceful degradation when daemon is offline.
- **Passive tool capture** -- New PostToolUse hook writes tool invocations to `~/.claudia/observations.jsonl` (~1ms per write, file append only). The daemon polls this file every 30s and ingests only relevant observations. Relevance filter checks 4 signals: tool name (gmail, slack, telegram, Rube apps), Claudia file paths (context/, people/, workspaces/), known entity mentions, and commitment language. Configurable via 5 new fields in `config.json` (`observation_capture_enabled`, `observation_capture_all`, `observation_relevant_tools`, `observation_relevant_paths`, `observation_ingest_interval`). Pure code operations are captured but filtered out during ingestion unless they mention known people or contain commitment language.

### Reliability Fixes

Three fixes for issues surfaced from daemon logs. All backward-compatible, no schema changes.

- **Overnight jobs now fire after sleep** -- APScheduler's `BackgroundScheduler` now has `misfire_grace_time=14400` (4 hours) and `coalesce=True`. Previously, the default 1-second grace time meant every scheduled job (decay, backup, consolidation, vault sync) was silently skipped when a Mac slept through the 2am-3:15am window. Now jobs fire immediately on wake if missed within the last 4 hours, with multiple missed runs collapsed into one execution.
- **Reduced log noise from summary memories** -- The content length warning threshold was raised from 500 to 800 chars. Legitimate summary-type memories (550-850 chars) no longer trigger "Long content" warnings. Hard truncation at 1000 chars is unchanged.
- **Fuzzy entity dedup on write** -- `_ensure_entity()` and `_find_or_create_entity()` now perform a fuzzy pre-check (SequenceMatcher > 0.90) before creating new entities. Name variants like "Kris Krisko" vs "Kris Krisco" (ratio ~0.92) match the existing entity instead of creating a duplicate. Only compares entities of the same type, skips deleted entities.
- **Expanded STOP_WORDS** -- Added ~55 common English words that spaCy misidentifies as entities ("drawn", "overall", "recently", "several", etc.). Prevents ghost entities from cluttering the graph.
- **Person entities require 2+ words** -- Regex-extracted person entities must have at least two words (e.g., "First Last"). Single-word extractions like "Metal" or "Drawn" are rejected. spaCy-identified entities are unaffected.
- 637 tests pass, 0 regressions, 22 new tests across 4 new test files.

- **LaunchAgent no longer bakes in --project-dir** -- The standalone background daemon now starts without a `--project-dir` argument. Previously, the plist could be identical across updates, leaving old daemon code running indefinitely even after `pip install --upgrade`.

v1.55.7 fixed FTS5 recall but broke vector/semantic search for every user. Three raw `sqlite3.connect()` calls skipped loading the `sqlite_vec` extension, and three KNN queries lacked the required `k = ?` constraint for JOINs. Net effect: 0% embedding coverage and silent fallback to keyword matching.

- **`load_sqlite_vec()` helper** -- Extracted the vec0 extension loading logic from `Database._get_connection()` into a standalone public function. Any raw connection that touches vec0 tables calls this one function. Eliminates the entire class of "forgot to load extension" bugs.
- **Backfill worker fix** -- `_backfill_worker()` now loads sqlite_vec on its raw connection. Previously it crashed within 60ms of starting because it couldn't query or write to vec0 tables. Degrades gracefully if the extension isn't available.
- **Index repair fix** -- `_check_and_repair_indexes()` now loads sqlite_vec on its raw connection. Previously it always reported 0 embeddings (triggering unnecessary backfill every startup). Improved exception handling distinguishes "no such table" from "no such module."
- **KNN `k = ?` constraint** -- Added `AND k = ?` to all three `embedding MATCH` queries in recall.py (`recall()`, `recall_episodes()`, `search_reflections()`). vec0's KNN queries require this constraint when JOINs are present because SQLite's query planner can't push an outer `LIMIT` into the virtual table scan.
- **Smarter briefing message** -- Embedding health check now reads `_meta['indexes_repaired']` to distinguish "backfill in progress" from "backfill never started." No longer tells users to "Start Ollama" when the real problem was a code bug.

### The Recall Recovery Release

After v1.55 consolidation, recall could return zero results despite all memories existing. Three-layer fix restores recall for affected users automatically on next startup.

- **FTS5 rebuild after consolidation** -- `merge_all_databases()` now rebuilds the FTS5 full-text index after merging. The migration's separate SQLite connection bypassed triggers that keep FTS5 in sync, leaving the index empty.
- **LIKE fallback fix** -- `_keyword_search()` now falls through to SQL LIKE when FTS5 returns 0 rows (not just on exception). Previously, an empty-but-functional FTS5 table returned nothing and the LIKE fallback never activated.

A single batch inference can no longer create a 0.9-strength relationship. Instead, inferred relationships start capped at 0.5 and must earn trust through re-encounter.
