📝 If the spec grows or shrinks (an edge case surfaces, scope needs narrowing, etc.), absorb it in
place with `ticket edit` on Scope/Plan/Analysis/Direction — no phase round-trip, no re-approval
ceremony. If the approved scope itself was the wrong premise, `ticket discard` it and start over.
