Include a claim matrix with:

- Atomic claim.
- Classification: confirmed, partial, stale, contradicted, or unverifiable.
- Evidence references and source type.
- Branch, timestamp, or applicability notes.
- Confidence and reason.
- Contradictions and unresolved questions.
- Suggested corrective work without execution.

Also include the shared changed-state declaration and handoff.
