### 2.4 Confidence Level Guidelines

**HIGH (90-100%):**
- Direct quote from INITIAL_PROMPT.md or CLARIFICATION_ANSWERS.json
- Verified by reading actual file (file:line)
- Explicit user requirement

**MEDIUM (70-89%):**
- Inferred from context but reasonable
- Based on project patterns (not explicit requirement)
- Related code suggests this need

**LOW (<70%):**
- Assumption without evidence
- "Seems like" or "probably needs"
- **→ DO NOT ADD TO TODO - Ask for clarification instead**

### 2.5 Rules for TODO File

**CRITICAL RULES:**
1. **MUST** include all 6 fields: Evidence (Source), Evidence (AI_PROMPT), Impact, Solution, Confidence, Action
2. **MUST** quote exact text from source files (not paraphrase)
3. **MUST** include file:line references
4. **MUST NOT** add items with LOW confidence (ask for clarification instead)
5. **MUST NOT** use vague language ("seems", "probably", "might")
6. **MUST** verify evidence by reading the actual file
7. Always UPDATE the TODO file, don't overwrite history
8. Keep track of which iteration found/processed each item
9. Mark items as `[x]` ONLY after actually updating AI_PROMPT.md
10. Categorize: [CONTEXT], [CLARITY], [COVERAGE]
