## Trigger

Use before writing project-native facts or decisions.

## Inputs

The relevant sources, task boundary, authority context, and intended durable output.

## Procedure

1. Identify the claim being made.
2. Locate the recoverable source artifact or explicit authorized answer.
3. Separate fact, inference, assumption, and unknown.
4. Record provenance and scope.

## Verification

- Every durable factual claim links to a recoverable source or is explicitly labeled as an assumption or unknown.

## Failure Modes

- Treating memory or generated synthesis as source truth.
