Confirm Stage C for plan review or Stage D for implementation/code review is activated and inherited. Review the requested plan, diff, commit, or branch read-only. Pass the CodeGraph gate; use CodeGraph for impact, LSP for exact contracts, and structural tools when detected. Compare the result with the active plan, decisions, acceptance criteria, and test evidence. When scope or risk justifies it, use task fan-out only when detected and reconcile all lenses into the shared review shape: P0–P3 priority, confidence, location, consequence, countermeasure, and a ship/block verdict. Run `ponytail-review` separately when recorded; it does not replace correctness review. Distinguish findings from questions. Do not fix code or perform external mutations.
