## Escalation triggers

If any of these happen, escalate into a full certificate:

- the answer affects merge safety
- more than one plausible explanation remains
- the issue spans multiple files or layers
- third-party semantics are involved
- the reasoning is beginning to branch

## Anti-patterns

Do not write:

- "read this file just in case"
- "probably this is the bug"
- "looks equivalent"
- "I think this helper does X" without evidence
- "done" while unresolved questions still directly affect the conclusion

## Acceptance Criteria

- Every file request is justified by a stated hypothesis
- Observations include locations and relevance
- Hypotheses are confirmed, refuted, or refined after each step
- Escalation triggers are checked

## Quality Gates

- Evidence quality is rated per step
- Trace completeness is rated per step
- Unresolved questions are documented

## Suggested Workflow

1. Define session metadata (task, question, target)
2. For each exploration step, state hypothesis before acting
3. Record observations after reading
4. Update hypothesis status
5. Decide to continue or stop
6. Escalate to certificate if triggers are met
