# Turn Docs Use exactly one Markdown turn doc per phase: ```text docs/implementation//turn-docs/.md ``` Record: - phase selection and accepted outcome - orchestration brief and later adaptations - discoveries and resolved open questions - implementation and delegation evidence - changed files and behavior - tests and quality gates - review findings and repairs - CI ownership, state, and evidence - PR and commit state - Beads mutations and follow-ups - plan amendments or replanning triggers - closeout and context worth retaining Do not create a separate reviewer turn doc. Keep the document useful as an execution record, not a transcript. Completion criterion: the phase can be understood and resumed from Beads, its phase doc, and this one evidence record without reopening every child context.
