   - `decisions.log` full history (keep only MANIFEST last-entry pointers)
   - Full blockers.md (keep counters only)
   - Full CHANGELOG.md
   - Scaffold templates not in use
3. Before next action, re-enter Tier 0 flow from CLAUDE.md § 1
